Tears in the pelvic floor can occur anywhere but the most common area is the perineum. This is because when the baby comes through, the area of the pelvic floor that has the best ability to stretch is the backside. On the front side, the pubic bone is pretty close to the vaginal opening so most stretch happens through that perineum. There are different grades of perineal tears. I am going to go over each of them so that if you have been told what grade you have, you know what that means. The greater the grade, the more scar tissue, the more layers affected, and the longer the healing process. But all tears will heal and normal pelvic floor function can be regained no matter what grade you have.

Grade 1 &ndash; perineal skin
Grade 2 &ndash; perineal skin and muscle
Grade 3 &ndash; perineal skin, muscle, and anal sphincter
Grade 4 &ndash; perineal skin, muscle, anal sphincter, and rectum
