Question four. Why was there no clarity? Because new requirements were not included in the training? Question five. Why were new requirements not Not Included in training, because standard operating procedures were not requested. Interesting, isn't it?

Example two. This is also an awesome example. And I love sharing this in every lecture where I discuss five why's. In this example, one of the washington dc monument was deteriorating.
