First project is typical of a veterinarian office and the data which they collect. In this case, information is entered by the office stuff into an Excel spreadsheet, and has a similar appearance to the earlier lecture where we enter student and advisor information. This Excel spreadsheet is called a form and at the beginning stage of creating a database. One of the first requirements is to identify all the forms presently used in the system that eventually will be entered into the database. study this Excel spreadsheet carefully. You may wish to freeze the screen and review each row or triple and column or field to get a better understanding.

There are four questions to complete. The first to deal with anomalies. Question three is related to energy Cheese. Remember our earlier analogy about sentences in a paragraph? that it must relate to one idea only. Similarly here, there was more than one idea or entity.

Can you spot all the entities? The question for us to draw a conceptual diagram, that is draw a box and at the top identify the name of the empty and below all the columns or fields in database terminology, then indicate how the entities are related with connecting lines between the boxes. Record your answers to the question on a separate piece of paper and answers will be provided in the next section of the course.
