The incident with H2S:


An oil worker operating a collapsed valve when exposed to H2S. The foreman tried to rescue him and was defeated, as was a third rescue worker. None of the rescuers had gas monitors or wore respiratory protection.
The remaining members of the crew wearing self-contained breathing apparatus eventually removed the injured workers from the area. The third worker survived, but the first worker and the foreman died.
